As Ask X3, your fleet-compliance assistant, here is what is required according to 49 CFR 107.803:
Before performing any cylinder inspections and verifications required by parts 178 and 180, a person must apply to the Associate Administrator for approval as an independent inspection agency (IIA).
To secure and maintain this approval, the applicant must meet specific criteria and application requirements:
* Independence: The applicant cannot manufacture hazardous materials cylinders, nor can they be controlled by or financially involved with any entity that manufactures them (except for providing independent inspection services). They must also state they will perform their functions independently of cylinder manufacturers and owners.
* Application Details: The applicant must submit an application following § 107.705 and include:
* The name and address of each facility where tests and inspections will be performed.
* A detailed description of the inspection and testing facilities.
* A detailed description of their qualifications and ability to perform/verify the required inspections (or those under a special permit).
* The name, address, and principal business activity of anyone with a direct or indirect ownership interest greater than 3% in the applicant, its subsidiaries, or divisions.
* The name and qualifications of each proposed inspector who will certify inspection and test results.
* An identification or qualification number for each inspector supervised by a certifying inspector.
* If the principal place of business is outside the U.S., a copy of the designation/approval from the country's Competent Authority.
* The signature of the person certifying the application and the date signed.
* Facility Inspections: The applicant must allow the Associate Administrator (or a designee) to inspect their facilities and records upon request, and the applicant must pay the cost of this inspection.
