Do truck drivers need the CMV Driver Medication Form?
Editorial correction · sources checked 2026-10-02
Short answer
FMCSA's CMV Driver Medication Form, MCSA-5895, is optional. It is different from a medical examiner's certificate and does not itself establish that a driver is medically qualified.
In plain English
A medical examiner may use the form to obtain information from a prescribing clinician about medication and its effects on safe commercial driving. The examiner evaluates qualification under the applicable medical standards. Taking a prescribed drug does not automatically make a driver qualified or disqualified, and an optional form does not replace the required examination or certification.
What to do
• Tell the medical examiner about your medications and relevant treatment.
• Ask the examiner whether information from your prescribing clinician is needed.
• Do not stop or change medication based on this page; discuss that with your clinician.
